Bremelanotide, commonly referred to as PT-141, is a cyclic synthetic peptide composed of seven amino acids. It functions as an agonist of melanocortin receptors, similar to the naturally occurring hormone alpha-melanocyte-stimulating hormone (α-MSH). PT-141 is a metabolic derivative of Melanotan II and has been studied primarily for its potential effects on sexual function and reproductive physiology.

PT-141 interacts with melanocortin receptors MC3R and MC4R, influencing central nervous system activity:

• MC3R: Expressed primarily in the brain, especially the hypothalamus, MC3R is involved in energy homeostasis, metabolic regulation, and may influence feeding behavior, including appetite and nutrient intake. MC3R may also modulate the activity of other melanocortin receptors in the region.

• MC4R: Central to appetite regulation and energy expenditure, activation of MC4R may contribute to increased energy output and weight regulation. Research also suggests a role for MC4R in reproductive function, including modulation of penile tissue function and sexual behavior.

Experimental studies indicate that PT-141 binding to MC3R and MC4R can activate hypothalamic neurons, which in turn may stimulate sexual arousal in animal models. These findings have positioned PT-141 as a potential therapeutic agent for sexual dysfunction, with ongoing research exploring its broader physiological effects.
