Tesamorelin is a synthetic 44-amino-acid peptide structurally analogous to growth hormone-releasing hormone (GHRH). The peptide features specific N-terminal and C-terminal modifications, which are believed to enhance stability and resistance to enzymatic degradation compared to natural GHRH. These modifications include an acetyl group at the N-terminus and a trans-3-hexenoic acid group at the C-terminus, resulting in the chemical designation N-(trans-3-hexenoyl)-[Tyr1]hGRF(1–44)NH2 acetate.

Overview

Tesamorelin acts on GHRH receptors in the anterior pituitary, stimulating the production and release of growth hormone (GH). GH, in turn, influences multiple tissues, including hepatocytes, where it promotes systemic and local synthesis of insulin-like growth factor-1 (IGF-1). IGF-1 is considered the primary anabolic mediator of GH, supporting cell growth, tissue repair, and inhibition of programmed cell death, while GH itself exhibits lipolytic activity, particularly in abdominal and visceral fat.

Mechanistically, Tesamorelin’s interaction with GHRH receptors may activate adenylate cyclase, increasing cAMP levels and subsequently activating protein kinase A (PKA). This signaling cascade can enhance GH secretion from somatotroph cells. Clinical studies suggest Tesamorelin can increase overall GH levels by ~69%, mean pulse area by ~55%, and IGF-1 levels by ~122%, without significantly altering pulse frequency or peak GH levels.

Through its structural modifications and targeted receptor activity, Tesamorelin is designed to provide enhanced peptide stability and potent stimulation of the GH–IGF-1 axis, supporting metabolic regulation, tissue maintenance, and fat reduction.
